Data: Nursing Home Violations, Lawsuits and Ratings

The Chicago Reporter has compiled databases of violations investigated by the Illinois Department of Public Health, of civil suits filed against 92 Chicago nursing homes, and of the ratings of majority-black nursing homes for each state in the country where there are majority-black homes.

Click here to download a list of nursing homes facilities that have been determined by the Illinois Department of Public Health to have violated the state’s Nursing Home Care Act from the first quarter of 2005 to the first quarter of 2009. Each entry includes the following: the violation’s docket number and cause for investigation; the year and quarter in which departmental notice was given; and the nursing home name’s name, address, and zip code.

Click here to download a list of civil suits filed against 92 Chicago nursing homes in the Cook County Circuit Court. Each entry includes the following: the suit’s docket number; the nursing home named in the suit; the plaintiff’s name; the date filed; and the cause of action.
